Important: Medication Pickup Notice

If the student has any medication at school:

All medications in the school health offices are required to be picked up by a parent/guardian by 5/21 @12:00 cst. If not picked up by the date indicted, they will be disposed of according to FDA and DEA regulations.
If the student will require medication at school in the upcoming school year, please obtain the Parent-Provider Authorization (PPA) from your school's health office or from the LCBOE website following this link: https://www.lee.k12.al.us/parent-authorization-for-school-med
